Prevalence of Non-Alcoholic Fatty Liver Disease in Different Phenotypes of Obesity.

Abstract: Background: Obesity is an emerging health problem causing increase in morbidity and mortality, neither all these obese individuals exhibit metabolic abnormalities nor all non-obese individuals are metabolically healthy. With these observations, two subtypes of obesity have been identified, Metabolic unhealthy normal weight (MUNW), Metabolic Healthy obese (MHO). Metabolic Syndrome include individuals with deranged BMI and abnormalities including increased visceral adiposity, increased blood pressure (BP), deranged lipid profile, and impaired fasting blood glucose. Metabolic healthy obese (MHO) include individuals with deranged BMI but without features of metabolic syndrome. Metabolic unhealthy Normal weight (MUNW) includes individuals with normal BMI but features of metabolic syndrome. . According to earlier studies resistance to insulin and metabolic disorders are independently associated to NAFLD This study is to estimate the association of different obesity phenotypes and to correlate the incidence of NAFLD in these phenotypes , the diagnosis is based on radiological ultrasonography stud of liver and NAFLD scoring system .This study will estimate the prevalence of different phenotypes of obesity including metabolic syndrome, metabolic healthy obese, metabolic unhealthy normal weight and correlate the prevalence of NAFLD in these subjects using radiological protocol and will compare with NAFLD scoring system . Objectives: • To estimate prevalence of different obesity phenotypes in the study population. • To estimate NAFLD in different obesity phenotypes by USG of liver • To estimate severity of fibrosis in different obesity phenotypes by NAFLD Scoring. • To compare both the methods in different obesity phenotypes • To correlate NAFLD with different variables in all obesity phenotypes Methods:The study will be carried out in a Rural hospital setup in central India , and it will be a Observational Cross sectional study. Patients height, weight ,BMI, blood pressure , waist circumference , biochemical parameters such as fasting glucose, serum dHDL, triglyceride, SGOT, SGPT, albumin levels and platelet counts are considered to ascertain the patient into one of the phenotypes of obesity and estimate the NAFLD score using above parameters and correlate it with USG findings and predict the correlation between scoring and radiological findings . Expected Results: This study aims at estimating NAFLD in different obesity phenotypes using NAFLD scoring and Ultrasonography. The prevalence of NAFLD in different phenotypes of obesity is as follows according to previous studies- Prevalence of NAFLD in Metabolic syndrome was 82.5%, Prevalence of NAFLD in MHO was 47.8% and Prevalence of NAFLD in MONW was 8.5%.
