Assessment of Serum IL8 and TLR-4 Concentrations Associated with Pantoea spp. Persistence in Recurrent UTI Patients

Abstract:
|
Background: Recurrent urinary tract infection (rUTI) is a significant source of morbidity and considerable mortality. Cytokines as Interleukin IL-8 and TLR-4 are important inflammatory markers in bacterial rUTI.
Objectives: This study aimed to assess some immunological marker (IL-8 and TLR -4) in UTI especially with emergence of multidrug resistant bacteria among UTI isolates to understanding coordinated innate immune system to clear infecting pathogens
Material and Methods: A case -control study was conducted on 800 suspected patients with recurrent urinary tract infection attending to AL-Diwaniyah hospitals in both sex and different age. Urine specimens from patients were cultured on specific media, and then bacterial isolates were identified via morphological, biochemical and Vitek-2 systems. Patients divided into two group 30 pyelonephritis and 25 cystitis and depended on infected with pantoea spp, or non-infected, and according to risk factors. The serum levels of cytokine IL-8 and TLR-4 were assessed in different groups after received agreement from patients and ethical comments from hospitals.
Results: Current study demonstrate that pantoea spp. isolated at 8% of cases(no=55 isolate), Data illustrate the higher mean serum concentration of TLR-4 and IL_8 in patients (21.64, 83.79 pg/ml) while in control (5.64, 20.34 pg/ml) , a significant difference in TLR-4, IL-8 (p < 0.05) between cystitis and pyelonephritis patients, (19.54, 68.9; 23.89, 94 pg/ml), in compare with control (5.64, 20.34 pg/ml), highly increased in TLR-4, IL-8 levels in pantoea infected patients. Finally, the highest mean of TLR-4and IL8 concentration in long-term catheterization patients followed by the V.U.R. patients.
Conclusion: Data revealed the coordinate role between IL-8, TLR-4 respond to recurrent urinary tract infection and consider suitable marker to differentiate between pyelonephritis and cystitis
