The burden of living with cancer due to smoking in Indonesia: Years Lived with Disability (YLD)

Abstract: Background: Smoking is continously becoming public health threat. This study aimed to estimate the burden of cancers caused by smoking in Indonesia using years lived with disability (YLD). Methods: This research was a descriptive epidemiology research. Measure of years lived with disability (YLD) was estimated by epidemiological parameters of number of morbidity and smoking attributable fractions (SAF). The smoking attributable fractions (SAFs) were estimated using prevalence of smokers in Indonesia obtained from National Health Survey 2018 and Relative Risk (RR) of smoking for cancer obtained from recent study. The incidence of cancer due to smoking were estimated by using cancer mortality based on 2018 National Health Insurance data. The duration of disability was calculated using DISMOD II software proposed by WHO. Results: In 2018, tobacco smoking is attributable to about 23% cancer cases in Indonesia. The greatest YLD due to smoking in Indonesia were lung cancer (149.129 persons years), larynx cancer (29.869 persons years), and lip oral cavity cancer (10.871 persons years). For YLD among men, the top rank years lost were lung and liver cancers. In contrast, larynx and lip oral cavity were most burden among women. Overall, Indonesia burden of cancers attributed to smoking was 221,384 YLD. Conclusion: This study provides evidence about the number of years lived with disability of cancers attributable to smoking an important data for strengthening tobacco control policies in country of Indonesia.
