Epidural anesthesia alternative choice of anesthesia in cardiac patient scheduled for laparoscopic surgery to minimize the risk of cardiac injury

Abstract:

Laparoscopic cholecystectomy commonly performed under general anesthesia with controlled mechanical ventilation to overcome the path physiological changes of carbon dioxide insufflations, increase intra-abdominal pressure and manipulations of patient positioning during surgery. Recently, laparoscopic surgery is targeted to manage the elderly who they have limited cardiac reserve and those with high risk under general anesthesia, in these patients, regional anesthesia offering several advantages. The present paper aims to find out the applicability of epidural anesthesia in laparoscopic cholecystectomy (LC) in patients with compromised cardiopulmonary status who they are at risk to have their operation under general anesthesia or who they are unwilling to do their surgery under regional anesthesia. This study was approved that all patients have had stable vital sign (mean Blood pressure & heart rate) Mild to moderate shoulder pain which occurs in 42.8% of patients were treated by small dose of opioid. Epidural anesthesia is a good choice and reasonable type of anesthesia can be used for laparoscopic cholecystectomy in patient with compromised cardiovascular system & for those patients who they are unwilling to do their surgery under general anesthesia and provide good post-operative outcome
