Comparing the Effect of Open and Closed Suction Techniques in Terms o f En vironmental Contaminate, Cost, a nd Ventilator Associated Pneumonia Based on the New Definition o f National Nosocomial Infection Surveillance System ( NNIS

Abstract: Todays, using ventilators and tracheal tubes is vital due to the increasing number of patients requiring intensive care. Opening the airway requires tracheal tubes suction. Selecting the appropriate method of suction can reduce the adverse consequence. The present study aimed to compare the effect of the open and closed suction techniques in terms of environmental contaminate, cost, and Ventilator associated pneumonia based on the new definition of NNIS. The present study was a quasi-experimental clinical trial that was performed for two months from November 22, 2017 until January 20, 2017. All intubated patients hospitalized for more than two days in intensive care units (ICUS) and general medical department of Tehran University of Medical Sciences where closed suction system (Vitaltec) was used were compared to all intubated patients hospitalized for more than two days in other units which has an open suction system of the same center in terms of ventilator associated pneumonia based on the new definition of NNIS and Centers for Disease Control and Prevention (CDC), environmental contamination and cost rate. The obtained results indicated that the incidence of pneumonia in the closed method was among 17 patients (43.58%) and in the open method was among 22 patients (56.41%). Comparing the types of suction in terms of incidence of pneumonia was statistically significant (P = 0.001) using Chi-square test. Data analysis indicated a significant difference between the type of suction and the microorganism causing environmental contaminate (P = 0.047). In addition, based on the obtained results, using the closed suction for patients was more cost effective than the open suction, which was significant between the two suctions (P <0.05). Considering the advantages of closed suction compared to the open suction, the closed one can be replaced by the open suction in terms of the incidence of pneumonia, economic cost, and environmental contaminate.
