A Study of Ultrasound Guided Examination Of Peripheral Nerves With Electrophysiologic Correlation In Leprosy

Abstract: Leprosy (Hansen`s disease (HD); Hanseniasis) is a chronic disease affected by Mycobacterium leprae (M. leprae), infectious in some cases, and affecting the peripheral nervoussystem, skin and certain other tissues. Ancient society regarded leprosy as a consequence of sin, and due to its disfiguring nature and the fear of spread, patients were often ostracized by their community.To study the Ultrasonographic Cross Sectional Area [CSA] and fascicular changes of Ulnar nerve in patients newly diagnosed with Hansen’s disease. The Lepra bacillus has a strong predilection for Schwann cells of the peripheral nervous system. It invades and colonises the Schwann cells leading to inflammation, destruction and irreversible fibrosis of the peripheral nerves, thus causing deformities and disabilities. Early detection of nerve affliction can help to minimize long term sequelae. This study aims to appraise the degree of peripheral nerve involvement in Hansen’s patients clinically and ultrasonographically, along with nerve conduction studies to assess the degree of motor and sensory deficit. Use of electro-diagnostic and radiological aids in addition to careful clinical examination, can be of great help in the early detection of peripheral nerve involvement and consequently minimize the degree of nerve injury caused by the disease.
