Effect of Wiper Exercise and External Rotation Exercise in Side-lying on the Muscle Activity and Thickness of the Infraspinatus

Abstract: The purpose of this study was to investigate the effects of wiper exercise and side-lying external rotation exercise with slight abduction on the thickness of the infraspinatus, using sonography imaging and muscle activity of external rotators for selective muscle activation of the infraspinatus. Twenty healthy participants were recruited for this study. The participants performed two types of shoulder exercises; the wiper exercise and external rotation exercise in side-lying, which were randomly arranged. Surface electromyography was used to measure the muscle activity of the infraspinatus, posterior deltoid, and middle deltoid of the dominant side. The sonography measurement was used to compare the thickness of infraspinatus during two exercises. Paired t-test was used to compare the thickness of the infraspinatus and muscle activities between the two exercises. Cohen’s d was calculated to measure the size of the mean difference. The muscle activity of infraspinatus and a ratio of infraspinatus and posterior deltoid were significantly greater in wiper exercise than external rotation exercise in side-lying (p< 0.05). The muscle activity of posterior deltoid was significantly greater in external rotation exercise in side-lying than wiper exercise (p< 0.05). The thickness of infraspinatus was significantly greater in wiper exercise than external rotation exercise in side-lying (p< 0.05). Effect sizes for activity of infraspinatus, ratio of infraspinatus and posterior deltoid, and thickness of infraspinatus were medium (d=0.66~0.78) This study recommends the wiper exercise is a good indication for the selective and functional strengthening of the infraspinatus muscle while minimizing the posterior deltoid for shoulder dysfunction.
